ShareW-9 and OACAC Address

Gearing up for Membership renewal, GWI and Articulation may mean that you will be sending a payment to OACAC. Many business offices require a W-9 Form from OACAC in order to process payments. To ensure payment to OACAC, please provide the attached pdf file to your business office.

As a reminder a purchase order does not constitute payment. Invoices and/or receipts are sent only one time and most often via email or by logging on to www.oacac.org. Your invoices and/or receipts should be included with the W9 when you request payment from your business office. Duplicate invoices/receipts are not readily available so please save your copies and use our great web site!
